been accredited by CARF for a period of three years for the following programs:

This accreditation will
extend through June 2005. This achievement is an indication of your
organization's dedication and commitment to improving the quality of the lives
of the people receiving services. Services, personnel, and documentation
clearly indicate that present conditions show an established pattern of total
operation and are likely to be maintained or improved in the foreseeable
future.
Your organization should take pride in achieving this high
level of accreditation. CARF will recognize this accomplishment in its listing
of organizations with accreditation, and we encourage you to make this
accomplishment known throughout your community. Communication of this to
referral and funding sources, the media, and local and federal government
officials will enhance their respect for and trust in your organization and its
capabilities and may increase referrals. Enclosed are some materials which will
help you to publicize this achievement.
The survey report is intended
to support a continuation of the quality improvement of your programs. It
contains comments on your organization's strengths as well as suggestions and
recommendations. A quality improvement plan demonstrating your efforts to
implement the survey recommendations must be submitted within the next 90 days
to retain accreditation. Guidelines and the form for completing the plan are
enclosed for your use.
In about five weeks, your organization will be
sent a Certificate of Accreditation. In order to be certain that our
information is correct, it is important that you complete and return the
enclosed Certificate Order Form promptly. Please note that you may use this
form to order additional copies of the certificate. I
f you have any
questions regarding your organization's accreditation, you are encouraged to
seek support from Lisa Palmer, MPH, Resource Specialist in the Behavioral
Health Customer Service Unit, at extension 129.
We encourage your
organization to continue to fully and productively use the CARF standards on an
ongoing basis. We commend you for your commitment to achieve accreditation and
your consistent efforts to improve the quality of your programs. We look
